The issue is for whatever reason you have /Volumes/Mac OS X Base System/ mounted, or stuck there, and myhack was not able to unmount it, thus it was unable to mount the InstallESD and BaseSystem correctly and that is what caused the failure. Additionally I suspect based on what I see on your screen that you do not have a legitimate copy of mountain lion which migh also be a source of problems. If you do not have an apple developer account, you should not be attempting to run mountain lion until it is released publicly in the app store. I do not support or condone piracy.
