Perhaps platform-id 9 works better for your system. Restore the original DSDT and the FB I patched for 10.9.5.
Then try booting with -f again, see if there's any difference.
No difference....

I seem to have the same issue with my system with nvidia/HD 4000. Booting with -f fixes it.
btw brightness and toucpad didn't work after your DSDT

backup your dsdt and FB and try these. Repatched to platform-ID 4

Looks like it works with values 4 and 9. Tried feom 0 to 11. Display from NVIDIA and Video is IntelHD4000USB will work with the patched DSDT. Right now, we're trying to figure out the correct value to get full QE/CI for your display. Then I could repatch your DSDT.
Have you try the different value? from 0 to 11

Could be a DSDT issue. Do you have same CPU as the OP?
Try same boot flags but with different FB value

I forgot to say that I have 1 SSD (Crucial_CT128MX100SSD1) and pulled off my Intel WIFI
Other hardware are the same as in the guide

It's working on 10.9.5 now
But sleep, wake and sys start are very slow. Even after cache repair. When system starts in -v mode I see message:
ioapic version 0x20 vectors 64 87
And it freezes start on 35 - 40 sec. What do you think is there a solution?

You probably could update FakeSMC and GenericUSBXHCI if USB 3.0 port is not working, other than that, it looks fine.

Sorry if this response is a little late, but it might help others reading this.
The DSDT was already patched to disable the Nvidia and include the patch for HD 4000.
I just notice the bootpack from the guide is for 10.9 which include a patched FB.
If you're installing with 10.9.4 or 10.9.5, you'll lose QE/CI with the included FB patched for HDMI. You'll need a new patched FB.
I have the issue for my E6430 with HD 4000. Restore with the vanilla kext and you should have QE/CI again but without HDMI audio.
Let me know which version of Mavericks you're installing and I'll patch it for you.
Note: not sure why the author install kexts to S/L/E rather than in E/E since he's using myHack method but I can validate that you could install all the patched kext to E/E and run myHack/myFix (quick) without an issue.

Hi, I followed that guide too I have some problems:
1.When computer goes to sleep mode if there are any USB devices - he wakes up
2. System start > 1.5 min
3. Wake from sleep(without USB devices plugged in > 1min)
My Mavericks version is 10.9.2
