So you're saying that with Apfsdriversloader.efi added, you don't get the Clover GUI?
With Legacy Option ROMs enabled, you'll get 2 sets of boot options: Legacy and UEFI
Make sure you select the USB under UEFI
-
Make sure you have memory in the 1st slot if you only have 1 stick.

then try abm_firstpolldelay=20000

Remove OsxAptioFix2Drv-64.efi, you already have AptioMemoryFix.efi ( they are mutually exclusive) in /EFI/Clover/drivers64UEFI.
Seems all the drivers have different dates. Why not just install latest Clover v4844

You're running version .2.6.3
Go ahead and update CodecCommander to latest version 2.7.1 and try again.

Boot to BIOS, select load Default, apply.
Then go back and set it to

- Legacy Option ROMs enabled
- Secure Boot disabled
Try again. Post screenshot of error

In regards to the USB-C DP out, the frequencies are locked to 59.9 / 1920 x 1080. This is a 60hz native monitor with a much higher resolution. Is there something I can tune to unlock it?
Try booting with -igfxnohdmi
See here for more boot options

Can you post new IOReg file with USB-C DP monitor and HDMI monitor connected?

Remove USBPorts-Exx50.kext since you now have a new USBPort.kext
What are these kexts for?
- daspi.kext
- FTDIKext.kext
- intelhaxm.kext
- SiLabsUSBDriver64.kext
- ProlificUsbSerial.kext
- usbserial.kext
See here under Post Install to set the Fn keys for Brightness control
The kexts in my files for Touchpad has generic functions only. You'll have 2 fingers scrolling, 2 fingers right click, and click to drag.
The latest kexts for Alps touchpad right now for this model is not good and jumpy.Try it if you like

I assumed you know how to install since you already have El Capitan installed, you just have to update your kexts and Clover loader as I stated.
You can follow above guide if you're using Clover.

You'll need to rebuild the USBPort.kext. See here

Highest you can install for SandyBridge is HighSierra. Mojave dropped support to HD3000. Other was able to install Mojave but requires using old files from High Sierra.
You can update your files prior to upgrading like Clover loader and kexts to the most recent.
-
Just enable Legacy Option ROMs in the BIOS will fix that.

please post debug file

You forgot to add HFSPlus.efi to /EFI/Clover/drivers64UEFI with my files
