Add FakePCIID_BCM57XX_as_BCM57765.kext to /kexts/Other, remove BCM5722D.kext. Reboot to see if it helps

Did you keep the other folders, other than replacing those 3 file and folders attached? Make sure you have HFSPlus.efi in /EFI/Clover/driver64UEFI

Post the result after you ran the rebuild cache here for review. AppleHDA? You should have the vanilla AppleHDA in /S/L/E, but not in /L/E

USB issue. Chameleon and Clover files differ. Attach your Clover folder for review. Mainly, you need the USB_injector in /EFI/Clover/kexts/Other, SMBIOS must be set to MBP7,1 See here for Config file

Don't think you're booting from the same EFI folder you attached on post #7. The Config file points to theme Latitude. The wallpaper above definitely is something else. So the 3rd selection is Macintosh Sierra, what is the first selection on the left? The 2nd and 4th are Recovery HD

Sorry, you are dual booting El Capitan with what? You didn't specify on the post, I was just assuming Windows.

That's because it's for system with HD 4000 only, not for system with HD 4000 + NVS5200m.
