try this:
Delete the following
/Library/Preferences/com.apple.PowerManagement*Reboot and set to these settings:
sudo pmset -a hibernatemode 0
sudo rm -f /var/vm/sleepimage
sudo pmset -a hibernatefile /dev/null
sudo pmset -a standby 0
sudo pmset -a powernap 0
sudo pmset -a autopoweroff 0 -
Here you go, give this a try.

Select config-intelwifi and rename it to config.plist if you have the Intel/BT combo.

Try my HP EliteBook 840 G3 EFI, basically the same hardware, just use the config-intelwifi.plist for your Intel wifi card.
Might have to create your own USBPorts kext but we could work on that afterwards.

Just open a command prompt in Windows and run this command reboot. Set the time again if needed
Reg add HKLM\SYSTEM\CurrentControlSet\Control\TimeZoneInformation /v RealTimeIsUniversal /t REG_DWORD /d 1

Sorry, not familiar with that external USB drive. The internal WD is supported.
Try a different external SSD.
It could also be that the USB ports was not detected or lost connection when system is about to initialize.

According to the thread, USB-C works. HDMI might be hardcoded to the Nvidia which is disabled since it's not supported.

Your signature shows you're installing to an external SSD drive but what is the model of the internal drive?
It might be what's causing the crash, in that case it can be disable via SSDT patching.

System is a Skylake with HD 515, not Kabylake nor WhiskeyLake.

MacBookPro12,1 is for Broadwell, you should go with MacBookPro11,4 for Monterey
You have too many patches doing the same thing, use one or the other, not both or all.
- SSDT-PM.aml or SSDT-plug.aml
- SSDT-data.aml with CPUFriend.kext or
- CPUFriend.kext with CPUFriendDataProvider.kext

As stated by Hervé:
The include USBPorts kext is hardcoded. So if you change the SMBIOS, make sure to edit the USBPORT' info.plist to match it.

You probably updated the driver and opencore.efi, but no the config.plist. You can't mix and match. Should all be from the same version.

For Monterey you should use latest OC and Kexts

If anything else, do post your system's specs. Those laptops exist in Skylake and Kaby Lake models...
