You could create a bootable Dos disk or a WinPE and run the bios update from there.

Could be a USB issue or installer is corrupted. Try recreating.

For the Nvidia card, the BIOS version is not critical. Only with HD4000 that you need to keep it at A12 or below. Make sure to disable (uncheck) Optimus for your setup. -
Just its content Download EFI_Clover -> rename it to EFI Download E6220, copy the content and place it in /EFI/Clover So now ACPI/Kexts and Config.plist is inside /EFI/Clover

Ok, then it's confirmed. With that resolution you need to use E6220 bootpack, but make sure you follow the bootpack instruction guide correctly Note: Bootpack (EFI) consists of combining the Generic EFI_Clover + the model you're downloading!!!

With that bootpack, it's for Nvidia NVS5200m only. You need to disable Optimus in the BIOS. -
Are you sure about HD3000 and not nvs4200m with Optimus?

So the SD Card Reader is working? Is the Display showing up as HD 4400 now or still HD 4600? Sleep working? Sleep with LID Close? Wake with LID open?

It makes no difference because you still have 0x0a260006 patched in the DSDT. Remove that and you should be able to see some changes with the Config.

So if you put them all back, it'll work? If that's the case, it could be you're using the wrong ig-platform-id. Try experimenting with different one and matching up the HDMI-audio patches. ie if ig-platform-id = 0x0a2e0008 works then try HDMI-audio, port 0204(12), 0x0a2e0008, 0x0a16000c, 0x0a2e000d, 0x0a26000d Intel HD 4600/5000 (Azul) 0 = 00000604 1 = 0000060c 2 = 00001604 3 = 0000160a 4 = 0000160c 5 = 00002604 6 = 0000260a 7 = 0000260c 8 = 0000260d 9 = 02001604 10 = 0300220d 11 = 0500260a 12 = 0600260a 13 = 0700260d 14 = 0800260a 15 = 08002e0a 16 = 04001204

Launch this and select save. Compress and upload it when you have a good working video/audio adding the working patch.

Ok, perhaps the version of Clover you have is not reading the specific switch (flag) and enable it anyway. Now you need to determine which one actually enables your HDMI. Try adding 1 at a time.

Here. I removed all the HDMI-audio patches you added. Let's see if HDMI audio still works. rename it to Config.plist config-fix.plist.zip

You see the 'Disabled' instead of 'Enabled'. That means HDMI audio is not configure. So I'm confuse that it works. Those 2 kexts are for HD4400 graphics, not related for HDMI audio.

Did you install AppleHDA_ALC236.kext to /S/L/E or /L/E and then fix permission and rebuild cache?

Are you sure that the config above works? Do you have a patched kext for HDMI video? I looked at your new config. You removed the patched I added and replaced with multiple HDMI-audio patches but they are all disabled.

DW1390 should work OOB, if you want something faster, try DW1550, but will require kexts and some patching. Here use this Replace the file and folders to your current Clover folder. Your IOReg and screenshot didn't show it has a SD card reader though.

The USBInjector I linked in post #4 should work, you just need to keep smbios to MBP8,2.
