That's because it's for system with HD 4000 only, not for system with HD 4000 + NVS5200m. -

Delete "macOS Install Data" folder on the root and retry again

Try removing SSDT-3.aml in ACPI/Patched or during boot, in Clover Option->ACPI->tabledropping->check SSDT-3.aml

Don't use MBP13.3, this kills HDMI. Either use MBP13.1 or MBP13.2. Make sure Legacy Option ROMs is disable, maybe causing the sleep issue if enabled. -

Default will for HDD will be HFS. Might be slow if you convert to APFS

what patches had you applied so far? For Fn keys re-assignment, use Karabiner-Elements.app and set the Fn keys Try enabling FixShutdown_0004 for reboot issue. If you want DSDT patching, extract new raw files by executing F4 during Clover boot. Compress and attach the Origin folder.

in Other folder you have 2 versions of VoodooPS2Controller*. Remove the one on top. Do you have the same kexts in /EFI/Clover/kexts/Other? On my guide, you should have all kexts in Other, only kexts in Other/LE should be in /Library/Extensions. Could be having duplicate is causing the issue...
