make sure you have HFSPlus.efi in your clover drivers folder. if you used APFS as the drive format instead of HFS plus then you need APFS.efi driver in clover drivers. if it's a core storage volume then you need to revert it using terminal.

well the LE folder stands for Library/Extensions. trackpad kexts might be in there. did you transfer them. -

are you sure CSR and SIP are disabled in config.plist? -
sounds like 3 issues one being corrected by hfsplus.efi, the windows issue can be either you where set up with a MBR windows install or you lost your EFI/microsoft boot folder. not being able to use your macos partition that one confounds me since you state it boots? so maybe im misreading the issue. it seems 2 out of the three issues have to do with bad EFI issues

did you ever add HFSPlus.efi to /EFI/Clover/drivers64UEFI like jake mentioned ? that should have fixed the not seeing mac os a clover boot option.

hmm your right I just checked ark intel and it does not specify a intel GPU for your clarksfield CPU. as for the NVIDIA Quadro FX 3800M it seems it is a legacy product so not sure if it is compatible with your OS. -

why would you have issues installing a legitimate dell bios from there download matrix? -
you should be able to just upgrade it by installing it but confirm that the options are still correct while installing

what version number is your rehab man ps2 kext? version 1.8.27 had issues for me and did not work but 1.8.28 and 1.8.26 do work on my synaptic system. -
I found web drivers but it says for "el cap" http://www.nvidia.com/download/driverResults.aspx/100824/en-us

you can also try the Nvidia lilu plugin if you think it's enabled. or try nv-disable=1

have you tried booting with -s flag and following the terminal instructions for fsck
